Forms & Fees
After scheduling an appointment, you will receive access to secure electronic intake forms through the patient portal.
These forms can be completed online prior to your visit. If needed, forms may also be completed in the office before your appointment.
Fees
This is a private-pay practice.
Fees reflect the time and depth of the clinical work involved:
Initial Psychiatric Evaluation (60 minutes): $500
Follow-Up (20–25 minutes): $250
Integrated Psychiatric Care (45 minutes): $300
Depth Psychotherapy (60 minutes): $350
Extended Psychotherapy (120 minutes): $650
Payment is due at the time of service.
Insurance
This practice does not bill commercial insurance.
A superbill may be provided upon request for possible out-of-network reimbursement.
