If you have Medicare, please do not use the online scheduler to book an appointment. To ensure proper compliance and coverage of your visit, please reach out to our office first, or securely submit your insurance information for verification. Please send a copy of your Medicare card, your secondary insurance card (if applicable), and a copy of your driver's license or valid photo ID via text message to our secure line at (805) 889-0709 or by email to info@drjazi.com. Our office will contact you with the next steps.
